عدنا بعض ال principal للaccess opening

اول شي قبل مانمبلش نحفر ونسوي cavity ضروري ناخذ اشعة ليش ؟؟

السبب حتى ينطي ال size للقنوات ممكن ماينطي الحجم بسكل دقيق لكن ممكن اقل ب١ ملم تقريبا

shape شكل القناة شنو
او اذا الroot بي انحناءات يبين عدنا بالاشعه حتى الطبيب يكون عنده داريه بالموضوع

نقطة مهمه

من نجي نحفر ونوصل للpulp chamber لازم نعرف شكد حجمه لان اكو اختلافات بالبلب جمبر من سن الى اخر ومن شخص الى اخر فحتى نعرف حجم البلب جمبر نخلي البير ع الاشعه الي اخذناها ونشوف عمق البلب وع هالحساب نعرف شكد ندخل بالبير



ثانيا نشيل كل التسوسات الموجوده قبل لا نفتح البلب لان ممكن المايكرو اوركانزم راح تدخل للcanale

السؤال هنا احنا اول وتالي راح نشي الpulp ليش نقلق اذا دخلت مايكرو اوركانزم للcanal

السبب اثناء ال insturmation داخل القنواة نسحب هاي الاوركانزم للقناة وتوصل لل apical area من توصل لهنا حيصير periapical lesion لذلك نتجنب هاي الخطوة ونشيل كل التسوسات اول بعدين نفتح القنوات


نشيل الحشوة القديمة باكملها قبل لا نفتح القناة والسبب حتى نتأكد انو الapical area يصيرلها seal

1)Entrance is always gained through the lingual surface of all anterior teeth. The initial penetration is prepared in the exact center of the tooth above the cingulum (in the center of the middle third).

2) The initial entry in the enamel is done by a round bur no. 4 operated at a right angle to the long axis of the tooth. The guide for enamel penetration is that only the head of the round bur no. 4 should enter the tooth.

The direction of the bur is changed to be parallel to the long axis of the tooth, and drill until the entrance to the pulp chamber. We can know that we reached the pulp chamber when we feel a fall in the resistance to the bur, i.e. the bur falls into a space which is the pulp.


تغير اتجاه البير وصار موازي للlong axis للtooth وبعدين وكف لان حس وصل للpulp chamber

3)Remove the roof of the pulp chamber by working from inside the chamber towards the outside of the chamber (pulling motion).

نشيل الroof مال بpulp عن طريق حركة تلpulling motion بحيث نحرك البير من الداخل للخارج


4) Lingual shoulder is removed by moving the bur from inside towards the outside to give a continuous smooth flaring preparation. Lingual shoulder is a convexity inside the pulp chamber.

Finishing and funneling with a fissure bur. The final shape funnels down to the orifice of the canal and flare outwards.

Extirpate the pulp by introducing an instrument called the barbed broach in the root canal and by outward movement the barbed broach will catch the pulp and remove it from the root canal.

الحركة هي outward movement وبهالحالة حيل م الpulp لان بي فد مثلثات صغار تلزم الtissue وبالحركة حتسحب الpulp



7) Irrigation of the pulp chamber.

نغسل البلب بالقاصر


The pulp horns should be eliminated with a round bur no. 2 used laterally and
incisally, because if they remain, remnants of necrotic tissue would cause discoloration to the anterior teet


اذا بقه عدنا خصوصاً بالانترير تيت اي بقايا من الpulp راح يبين عدنا بعد الجلسه وبعد العلاج لون السن حيكون رمادي مائل للسود والسبب هو بقايا من الانسجه الميته مانشالت

اذا بقت اي بقايا من الpulp حيكون لون السن هيج بعد اتمام الحشوة بالكامل

من ال anomalies المهمه هن ٦ لكن بالواقع بعد اكو مثل ال fusion ماذكو بين سنين


ع العموم اول شي

1- dentinogenesis imprefecta
اول شي كتعزيف هو disorder اثناء الtooth development فيجيك السن بحالتين ياما اصفر او ازرق


الي حيصير بهاي الحالة شغلتين الاولى البلب كولش صغير والشغله الثانيه الcanal مسدوده

هنا البلب شبه معدوم

hyperparathyrodism كتعريف للحالة هي زياده بالكالسيوم level

لذلك راح نلكه البلب بي stone بيض بالاضافه الى loss lamina dura
