🔺Upper abdominal causes:
1- Perforated peptic ulcer
هنا البيشنت ممكن يكون عنده rigidity in RIF برضه لأنه الـ gastric or duodenal contents ممكن flow along Rt paracolic gutter
2- Acute cholecystitis
وهنا الـ pain حيكون in upper right quadrant and radiating to Rt shoulder

🔺Lower abdominal causes:
1- non specific mesenteric lymphadenitis
والمميز هنا أنه الـ tenderness shifts with change of postion
2- Meckel's diverticulum
وهنا ما بنقدر نؤكد الـ diagnosis إلا بعد الـ exploration
🔺Urological problems
1- Rt ureteric colic
وهنا الـ pain حيكون from loin to groin
2- Rt pyelonephritis
وهنا الـ patient ممكن يكون عنده fever with rigor، في حين في الـ appendicitis بيكون عنده mild fever
🔺Gyenocological causes
1- Ruptured ectopic pregnancy
2- Twisted ovarian cyst
3- PID

قواعد جمعتها ليك عشان تسهل عليك الـ appendicitis
🔹 ال pain اللي بيبدأ في around the umbilicus تاني يمشي Rt iliac fossa ده بيميز ليك ال appendicitis لكن خلي بالك ما كل العيانين ح يقولوا ليك السيناريو النموذجي ده
🔹 ال pain ممكن يكون فوق في ال Rt hypochondrial area في حالة كان عنده short colon or pregnancy .. ف قد يتشابه مع الcholecystitis
🔹ال pain يبدا قبل ال vomiting ، إذا حصل العكس فكر في gastroenteritis
🔹الـ fever ما بتكون high
🔹لو البيشنت قادر يشرب وياكل عادي مافي anorexia فكر في diagnosis تاني
🔹ممكن يكون في vomiting لكن ما كتير
🔹ما شرط يكون في High WBCs
🔺 إذا عيان ال appendicitis بقى عنده high fever with rigor فكر في ال appendicular abscess
🔺 إذا عيان ال appendicitis بقى عنده high fever, rigor, mild jaundice فكر في ال portal pyemia
🔺 إذا عيان ال appendicitis بقى عنده generalised abdominal pain, tenderness and rigidity دي peritonitis يا معلم مفروض يتنقل السييتر فورا ويتعمل ليهو peritoneal toilet
